Skilled Nursing
Skilled nursing is for seniors who need more care than assisted living provides, but who don’t need to be hospitalized. They may have been recently discharged from the hospital and need post-acute care to regain their strength and function, or they may need long-term care for a chronic condition. At Wesley Manor Health Care Center, we provide around-the-clock skilled nursing care.
Guided by compassion and advanced therapies, we’re dedicated to helping residents regain strength, confidence, and independence at every step. Through our partnership with HTS Therapy – a leader in senior care – we provide top-tier physical, occupational, and speech therapy to support each resident’s recovery and enrich their quality of life.
We prioritize residents from our vibrant community for placement in our Health Care Center. This commitment helps your loved ones stay close to friends and familiar faces, enhancing their comfort and emotional well-being. Our seamless transition process ensures they receive specialized care while remaining connected to their supportive network, making them feel at home every step of the way.

Short-Term Rehab
Wesley Manor Health Care Center offers comprehensive short-term inpatient rehab services tailored to help residents and patients regain strength, mobility, and independence, so they can return to their lives as quickly as possible. Through our partnership with HTS Therapy, we develop customized rehab programs that may include physical, occupational, and speech therapies to support each resident’s unique recovery journey.
In our Health Care Center, residents enjoy private rooms and access to skilled nursing care, including advanced services like intravenous medications, nutritional support, and attentive nurse monitoring following illness or surgery. Our team is here to provide compassionate, expert care every step of the way.
We involve family members in the process, educating them on how to support their loved one’s journey toward independence and preparing them for a smooth transition home, wherever that may be. With compassionate staff, holistic support, and tailored discharge planning, our short-term rehab services aim to provide a full spectrum of care for a successful recovery.
In our continuum of care model, we prioritize Wesley Manor Retirement Community residents for placement in our Health Care Center. Our seamless transition process ensures they receive specialized care while remaining connected to their supportive network, without needing to move off-campus for short-term rehab after injury or illness.

Outpatient Therapy
Wesley Manor offers outpatient therapy services designed to help individuals regain strength, mobility, and confidence while they continue to live at home. Our skilled team, in partnership with HTS Therapy, creates customized therapy plans – including physical, occupational, and speech therapies – tailored to each patient’s unique goals. With a focus on restoring independence and enhancing quality of life, our outpatient services provide convenient, expert care that supports recovery in a familiar, comfortable setting.
For added flexibility, seniors can schedule one or multiple therapy sessions per week, depending on their needs. This adaptable schedule is designed to fit seamlessly into each individual’s lifestyle and daily routine, ensuring that therapy complements – not disrupts – their lives. Whether you’re recovering from surgery, managing a chronic condition, or looking to enhance mobility, our skilled and compassionate team is here to guide you every step of the way.
