Wesley Manor provides both short-term/inpatient and outpatient rehab services on campus. Our unique resident-centered approach and skilled therapy team focuses on restoring safety, functional mobility and independence. Physical Therapy, Occupational Therapy, and Speech Therapy are offered for those needing short term rehab-to-home in our private rooms, or others who require long-term rehab on Wesley Manor’s campus. Outpatient therapy is also available. Rehab may be appropriate for those who have had a recent hospital stay and who need intensive care such as intravenous medications, tube feeding to maintain nutrition or nurse monitoring following an illness.

Always a leader in Alzheimer’s and dementia care, a secure wing in our Health Care Center meets the special needs of individuals in the mid-to-late stages of dementia. Here residents move about freely, safely and securely within their own neighborhood. Cheerful dining areas enable residents to eat, socialize and visit with friends and family in comfort. Our goal is to incorporate each person’s individual preferences and customary routines into our daily living schedule.
Residents have access to a gated enclosed courtyard and patio, giving them a sense of freedom and lots of opportunities to enjoy the outdoors. All long-term care rooms are shared accommodations and have a half bath, built-in clothing storage and large windows providing natural sunlight. Wesley Manor residents from other areas of the campus receive priority placement to the Health Care Center. All beds in the Health Care Center are Medicare and Medicaid certified.

- Weekly visits by geriatric physician and geriatric psychiatrist; on-call 24 hours a day
- Regularly scheduled podiatry, dental, vision and hearing clinics
- Specialized meals, dietary supplements and snacks
- Certified Activities Director, licensed Social Worker and full-time Chaplain
- Regular entertainment, exercise and daily activities
- Barber and beauty salon
- Monthly Alzheimer’s Support Group for family and caregivers
Wesley Manor staff works closely with discharge planners, case managers, social workers and physicians from all hospitals and rehab facilities. Our goal is to improve patient outcomes, to reduce readmissions and enhance quality of life. We partner with Heritage Healthcare, a recognized leader in providing physical, occupational and speech therapy to seniors.

All potential admissions are subject to client needs and medical assessment by Wesley Manor staff, as well as bed availability at time of discharge from the hospital. Wesley Manor will only admit clients for whom we can provide the highest quality of care, in their best interest.
