Skilled Nursing &
Long-Term Care
Wesley’s Manor long-term care program includes a variety of services design to meet health and personal care needs, for varying lengths of stays. Here, our residents receive daily assistance with medical needs such as administering medications and non-medical needs such as dressing, eating and using the bathroom.
All long-term care rooms are shared accommodations and include a half bath, built-in clothing storage and large windows providing natural sunlight. Wesley Manor residents from other areas of the community receive priority placement to the Health Care Center.

Incoming residents receive a health assessment to help our skilled team create a personalized care plan to improve functional mobility and independence. All Health Care Center beds are certified for private pay, Medicare, Medicaid and select private insurance plans.

Health Care Center
Wesley Manor’s Health Care Center is a skilled nursing facility with a unique resident-centered approach. The Transitional Care Program is a great option for further recuperation and therapies after a hospital stay. Our caregiving teams work to enhance patient results, reduce the risk of re-hospitalization for older adults, and ease the return to everyday life. The Health Care Center is also a wonderful long-term care home for our residents that need 24/7 nursing care. Residents receive round-the-clock assistance with healthcare and activities of daily living (ADLs).
Rehabilitation
Services
Wesley Manor provides both short-term inpatient and outpatient rehabilitation services. Customized therapy programs help our residents and patients recuperate, regain strength and functional mobility, and independence to return to their lives quickly. Physical Therapy, Occupational Therapy, and Speech Therapy are offered are part of our short-term rehab-to-home program. These residents enjoy private rooms and have access to intensive care such as intravenous medications, tube feeding to maintain nutrition or nurse monitoring following an illness.

Professionals
The Wesley Manor team works closely with discharge planners, case managers, social workers and physicians from hospitals and rehab facilities. Our goal is to improve patient outcomes, to reduce readmissions and enhance quality of life. We partner with Encore Rehabilitation, a recognized leader in providing physical, occupational and speech therapy to seniors.

All potential admissions are subject to client needs and medical assessment by Wesley Manor staff, as well as bed availability at time of discharge from the hospital. Wesley Manor will only admit clients for whom we can provide the highest quality of care, in their best interest.
